Privacy Policy
Nicky’Sports Massage is a sole trader and Nicky Holbrook is the sole data collector and user.
This privacy policy will explain how Nicky’Sports Massage uses any personal data collected. Data is collected from Rehab Guru (online booking and client management system), paper Consultation and Consent forms and Newsletter sign-up.
1. Collection of data - You directly provide Nicky’Sports Massage with most of the data we collect and it is used to:- book and manage appointments online via Rehab Guru www.rehabguru.com, keep treatment records, contact digitally clients about their home care, payment and invoices, meeting insurance and professional standards expectations.
2. Information is collected to contact clients for communication, for information to understand medical, lifestyle and well-being history, in order to understand if there are contraindications or adaptions required for treatment, to understand Client’s objectives for treatment and to understand and assess the client.
3.Storage of Data - digitally this is accessible only via secure password. Paper copies of forms and shared medical information, is stored in A4 files in a cupboard out of sight in Therapy room.
4. Data is only shared with another person, if agreed verbally/and or in writing by Client, and it is recorded. With consent Data can be shared, for example for a referral to another Therapist, GP or another Health Consultant. Record kept of what information shared and why.
5. Data is kept for 8 years for adults (over 18years old) post a last appointment. For under 18s data is kept until they reach 21 years old (assuming their last treatment is prior to their 18th birthday). If there are mental health issues, information may need to be be kept longer or indefinitely but for at least 3 years after they are certified to have regained mental capacity.
6. Records when disposed are shredded.

Every Client is entitled to the following:
The right to access – You have the right to request for copies of your personal data.
The right to rectification – You have the right to request the correction of any information you believe is inaccurate. You also have the right to request to complete the information you believe is incomplete.
The right to erasure – You have the right to request the erasure of your personal data, under certain conditions.
The right to restrict processing – You have the right to request the restriction of the processing of your personal data, under certain conditions.
The right to object to processing – You have the right to object to the processing of your personal data, under certain conditions.
The right to data portability – You have the right to request that Our Company transfer the data that we have collected to another organisation, or directly to you, under certain conditions.
